ABSTRACT

La esofagitis necrotizante aguda es un trastorno poco común que puede ser causa de hemorragia digestiva alta. Predomina en el sexo masculino en la sexta década de la vida. El diagnóstico es endoscópico y muestra una mucosa esofágica de apariencia negra que afecta al esófago distal en toda su circunferencia y se detiene abruptamente en la unión gastroesofágica. Clínicamente suele presentarse con hematemesis y melenas, shock hipovolémico por sangrado masivo, siendo otras manifestaciones el dolor epigástrico, molestia retroesternal y disfagia. Se vincula a pacientes con antecedentes de enfermedad cardiovascular, alcoholismo, diabetes mellitus, desnutrición, hernia hiatal, estenosis gastroduodenal, cáncer, así como pacientes en shock, traumatizados, sometidos a cirugía mayor e inmunosuprimidos. El tratamiento se basa en fluidoterapia, inhibidores de la bomba de protones y suspensión de la vía oral, siendo controvertido el uso de antibioticoterapia. Su pronóstico es malo y dependerá de la gravedad de la enfermedad esofágica y del terreno del paciente, con una mortalidad de hasta el 36 %. Presentamos el caso clínico de un paciente de 81 años, hipertenso, que presenta hematemesis, confirmándose en la endoscopía una esofagitis necrotizante aguda, que evoluciona favorablemente con tratamiento médico.


Acute necrotizing esophagitis is a rare disorder that can cause upper gastrointestinal bleeding. It predominates in males in the sixth decade of life. The diagnosis is endoscopic and shows a black-appearing esophageal mucosa that affects the entire circumference of the distal esophagus and stops abruptly at the gastroesophageal junction. Usually, patients present with hematemesis and melena, with other manifestations such as epigastric pain, retrosternal discomfort, dysphagia, and hypovolemic shock. Almost all patients reported comorbidities: cardiovascular disease, alcoholism, diabetes mellitus, malnutrition, hiatal hernia, gastroduodenal stenosis, and malignant neoplasia; is related as well to patients with shock, trauma, undergoing major surgery, and immunosuppression. The treatment is based on fluid reposition, proton pump inhibitors and suspension of the oral route, the use of antibiotic therapy being controversial. Its prognosis is poor and will depend on the severity of the esophageal disease and the patient comorbidities, with a mortality rate up to 36 %. Case: A 81-year-old male patient with hypertension, who presented hematemesis, confirmed by endoscopy as acute necrotizing esophagitis, whose evolution was favorable with medical treatment.


A esofagite necrosante aguda é uma doença rara que pode causar hemorragia digestiva alta. Predomina no sexo masculino na sexta década de vida. O diagnóstico é endoscópico e mostra uma mucosa esofágica circunferencial difusa com aspecto preto que envolve quase universalmente o esôfago distal e para abruptamente na junção gastroesofágica. Clinicamente, geralmente se apresenta com hematêmese e melena, com outras manifestações sendo dor epigástrica, desconforto retroesternal, disfagia e choque hipovolêmico. Está relacionado a pacientes com histórico de doenças cardiovasculares, alcoolismo, diabetes mellitus, desnutrição, hérnia hiatal, estenose gastroduodenal e neoplasia maligna, bem como pacientes em choque, trauma, cirurgia de grande porte e imunossupressão. O tratamento é a medicação dietética higiênica baseada em fluidoterapia, inibidores da bomba de prótons e suspensão da via oral, sendo o uso de antibioticoterapia controverso. Seu prognóstico é ruim e dependerá da gravidade da doença esofágica e do terreno do paciente, com mortalidade de até 36 %. Apresentamos o caso clínico de um paciente hipertenso de 81 anos que apresentou hematêmese, confirmada por endoscopia como esofagite necrosante aguda, que evoluiu favoravelmente com tratamento higiênico-dietético e medicamentoso.

Introducción: La hemorragia digestiva alta representa una de las causas más frecuentes de morbilidad y mortalidad en los servicios de cirugía general y específicamente es la primera causa de mortalidad en el servicio, por lo que constituye una emergencia médico-quirúrgica. Objetivo: Identificar la existencia de un patrón estacional en la incidencia de hemorragia digestiva alta en época invernal y estimar la frecuencia de algunos factores de riesgo de esta enfermedad. Métodos: Se realizó un estudio descriptivo, de series temporales, de pacientes afectados por esa enfermedad que acudieron al Hospital Dr. Miguel Enríquez en el periodo comprendido entre diciembre de 2017 y enero de 2020. La muestra quedó conformada por 151 pacientes que presentaron como diagnóstico de ingreso hemorragia digestiva alta. Resultados: Predominó el sexo masculino y los mayores de 60 años. La temporada de mayor incidencia de esta complicación digestiva fue la invernal (diciembre, enero y febrero). Los factores de riesgo que predominaron fueron los hábitos tóxicos y la ingestión de AINES y ASA. La forma de presentación más frecuente fue la melena y la principal etiología la úlcera péptica duodenal. Conclusiones: Los casos con hemorragia digestiva alta predominaron en la temporada invernal y los factores de riesgo más frecuentes fueron los hábitos tóxicos y el uso de AINES en relación con el periodo estacional(AU)


Introduction: The High Digestive Hemorrhage represents one of the most frequent causes of morbidity and mortality in the General Surgery Services and it is specifically the first cause of mortality in our service. It constitutes a very frequent medical-surgical emergency. Objective: To determine the existence of a seasonal pattern in the incidence of HDA in winter and its relationship with risk factors. Methods: A descriptive and retrospective study was carried out with a longitudinal design in which patients affected by HDA were studied. These patients were assisted at "Dr. Miguel Enriquez Hospital" between December 2017 and January 2020. The study group was composed of all the patients who came to our emergency services with manifestations of bleeding from the upper digestive tract. The sample was made up of 151 patients who presented a diagnosis of HDA at the time of their admission. Results: The predominant sex was male and the age over 60 years old. The season with the highest incidence of this digestive complication was winter (December, January and February). The risk factors that predominated in our study were toxic habits and ingestion of AINES, ASA. The predominant form of presentation of the HDA were tarry stools, being the Duodenal Peptic Ulcer the main etiology. Conclusions: Cases with Upper Digestive Bleeding predominated in the winter season and the most frequent risk factors were toxic habits and the use of NSAIDs in relation to the seasonal period(AU)

RESUMEN Introducción: el sangrado del tubo digestivo es infrecuente en pediatría. Aunque la mayoría de las causas no ponen en peligro la vida, se requiere un enfoque que permita al médico realizar un diagnóstico temprano y valorar la severidad del sangrado, así como la selección de estudios complementarios diagnósticos y terapéuticos. Objetivo: valorar el comportamiento clínico que presentan los factores causales de sangrado digestivo infantil en el Hospital Pediátrico Provincial Docente de Pinar del Río, durante el periodo 2019- 2021. Métodos: estudio observacional, descriptivo y transversal en una muestra constituida por 49 niños, seleccionados mediante un muestreo aleatorio simple y que cumplieron con los criterios de inclusión. La aplicación de una encuesta permitió la obtención de información que dio salida a las variables estudiadas. Para el análisis de los resultados se utilizaron los métodos de la estadística descriptiva, como las frecuencias absolutas y relativas. Resultados: predominó el grupo etareó de uno a cinco años (44,9 %). Se identificaron como factores de riesgo fundamental la dieta inadecuada (43,3 %) y la constipación (40,0 %). Predominó el sangrado digestivo bajo (84,3 %), donde la fisura anal (24,2 %) y los pólipos de recto (16,1 %) fueron las causas principales. Conclusiones: se presenta con mayor frecuencia en el menor de cinco años, en la zona urbana, la rectorragia y la hematemesis fueron las principales manifestaciones clínicas, predomina en sangrado digestivo bajo con la fisura perianal y los pólipos como causas principales.


ABSTRACT Introduction: gastrointestinal tract bleeding is infrequent in pediatrics. Although most of the causes are not life-threatening, it requires an approach that allows the physician to make an early diagnosis and assess the severity of the bleeding, as well as the selection of complementary diagnostic and therapeutic studies. Objective: to assess the clinical behavior presented by the causal factors of digestive bleeding in children at Provincial Pediatric Teaching Hospital of Pinar del Río, during the period 2019- 2021. Methods: an observational, descriptive and cross-sectional study in a sample comprised of 49 children chosen by means of a simple random sampling and having the inclusion criteria. The application of a survey allowed the collection of information to analyze the variables studied. For the analysis of the results, descriptive statistical methods such as absolute and relative frequencies were applied. Results: the group aged one to five years was predominant (44,9%). Inadequate diet (43,3 %) and constipation (40,0 %) were identified as basic risk factors. Lower gastrointestinal bleeding predominated (84,3 %), with anal fissure (24,2 %) and rectal polyps (16,1 %) as the main causes. Conclusions: it occurs more frequently in children under 5 years old, in the urban area, rectorrhagia and hematemesis were the main clinical symptoms and signs, lower gastrointestinal bleeding predominated with perianal fissure and polyps as main causes.

Resumen Caso clínico: Se describe un caso clínico poco frecuente en un paciente inmunocomprometido con hallazgo histopatológico de infestación parasitaria. Es un paciente masculino de edad media que habita en zona subtropical con diagnóstico de enfermedad de Crohn tratado con corticoide e inmunomoduladores, presentaba dolor abdominal y anemia crónica de 1 año de evolución, analítica negativa para parásitos, reactantes de fase aguda normales, gastroscopia y colonoscopia previas (6 meses) sin hallazgos relevantes. Por la persistencia del cuadro clínico se repitieron los estudios endoscópicos en los que se visualizaron hemorragias subepiteliales con resultados histopatológicos de Strongyloides stercoralis. Conclusión: En el contexto de un paciente inmunocomprometido, en zona endémica y con evolución tórpida, debe obligar a realizar un diagnóstico diferencial en el que se debe sospechar siempre de infestación parasitaria. Aunque la endoscopia no se necesita para el diagnóstico de estrongiloidiasis, su intervención puede ser oportuna.


Abstract Clinical case: The following is a rare clinical case in an immunocompromised patient with histopathological findings of parasitic infestation. The patient is a middle-aged male who lives in a subtropical area and has a diagnosis of Crohn's disease treated with corticosteroids and immunomodulators. The patient presented with abdominal pain and chronic anemia for 1 year, with negative laboratory tests for parasites and normal acute phase reactants. Gastroscopy and colonoscopy were performed before the consultation (6 months) without relevant findings. Due to the persistence of the symptoms, endoscopic studies were repeated, finding subepithelial bleeding with histopathological results of Strongyloides stercoralis. Conclusion: In the context of an immunocompromised patient living in an endemic area and with a torpid evolution, a differential diagnosis should be made always suspecting a parasitic infestation. Although endoscopy is not necessary to diagnose strongyloidiasis, its use may be convenient.

Introducción: Las varices esofágicas son canales vasculares que unen la circulación venosa portal y la sistémica. Se forman como consecuencia de la hipertensión portal, predominantemente en la submucosa del tercio inferior del esófago. Objetivo: Caracterizar a pacientes con rotura de varices esofágicas según principales variables clinicoepidemiológicas y terapéutico-endoscópicas. Métodos: Se realizó un estudio observacional, descriptivo y longitudinal de 20 pacientes con hemorragia digestiva alta por rotura de varices esofágicas, los cuales recibieron tratamiento endoscópico con bandas elásticas en el Servicio de Gastroenterología del Hospital General Docente Dr. Juan Bruno Zayas Alfonso de Santiago de Cuba, de noviembre de 2016 a febrero de 2018. En el análisis estadístico se utilizó el porcentaje como medida de resumen y se aplicó la prueba de independencia de la Χ2 para identificar alguna asociación significativa entre variables. Resultados: Existió un predominio del sexo masculino (80,0 %) y del grupo etario de 51 a 60 años (40,0 %). La mayoría de los afectados presentó cirrosis hepática (60,0 %) como causa de la hipertensión portal y hematemesis (50,0 %) como forma de hemorragia digestiva alta; asimismo, el gran tamaño de las varices fue el principal factor de riesgo asociado a la rotura (70,0 %) y, luego de practicado el tratamiento endoscópico, no se produjo resangrado en la casi totalidad de los pacientes (95,0 %). Conclusiones: La aplicación de este procedimiento terapéutico resultó ser satisfactoria y no provocó complicación alguna.


Introduction: The esophageal varicose veins are vascular channels that unite the portal venous and systemic circulation. They are formed as consequence of the portal hypertension, predominantly in the submucosa of the inferior section of the esophagus. Objective: To characterize patients with break of esophageal varicose veins according to main clinical epidemiological and therapeutic-endoscopic variables. Methods: An observational, descriptive and longitudinal study of 20 patients with upper digestive bleeding due to break of esophageal varicose veins was carried out, they received endoscopic treatment with banding in the Gastroenterology Service of Dr. Juan Bruno Zayas Alfonso Teaching General Hospital in Santiago de Cuba, from November, 2016 to February, 2018. In the statistical analysis the percentage was used as summary measure and the chi-square test was applied to identify the existence of some significant association among variables. Results: There was a prevalence of the male sex (80.0 %) and the 51 to 60 age group (40.0 %). Most of the patients presented liver cirrhosis (60.0 %) as cause of the portal hypertension and hematemesis (50.0 %) as form of upper digestive bleeding; also, the great size of the esophageal varicose veins was the main risk factor associated with the break (70.0 %) and, after the implementation of the endoscopic treatment, there was no second bleeding in almost all the patients (95.0 %). Conclusions: The application of this therapeutic procedure was satisfactory and it didn't cause any complication.

Resumen Se presenta el caso clínico de un paciente masculino de 74 años quien ingresó con síntomas de sangrado digestivo alto, se realizó una endoscopia de vías digestivas altas y se encontraron dos lesiones en el estómago: una elevada de centro ulcerado y otra subepitelial, lo que llevó a completar los estudios con una ecoendoscopia gástrica que mostró una lesión mucosa que infiltraba la submucosa sugestiva de cáncer gástrico temprano y otra lesión subepitelial dependiente de la muscular, sugestivas de un tumor gastrointestinal estromal (GIST); los estudios de estatificación no evidenciaron compromiso metastásico, por lo que se llevó al manejo quirúrgico con la posterior confirmación histológica de los hallazgos.


Abstract We present the case of a 74-year-old male patient who was admitted with symptoms of upper digestive bleeding. Endoscopy of his upper digestive tract found an ulcerated lesion and a subepithelial lesion in his stomach. Complete studies including gastric endoscopic ultrasound showed a mucosal lesion infiltrating the submucosa which was suggestive of early gastric cancer as well as a subepithelial lesion on the muscle that was suggestive of a gastrointestinal stromal tumor (GIST). Staging showed no metastatic compromise, so surgery was performed, and histology subsequently confirmed the findings.

ABSTRACT

Background: Upper gastrointestinal bleed (UGIB) and dyspepsia are the commonest indications for an upper GI endoscopy (UGIE), which has the potential to provide both diagnostic and therapeutic intervention. Alarm symptoms in patients with dyspepsia need proper evaluation.Methods: In an observational hospital-based study, 5117 patient undergoing upper GI endoscopy were evaluated at Mahatma Gandhi Medical College and Hospital, Jaipur, Rajasthan, India. Detailed clinical and endoscopic profile was evaluated for subjects with dyspepsia and UGIB. Statistical analysis was done using SPSS version 21.0.Results: Dyspepsia (2887, 56.41%) followed by upper GI bleed (1124, 21.97%) were the most common indications for UGIE. In subjects presenting with UGIB, most patients had both hematemesis with Malena (48.04%), 48.93% were chronic alcoholics and nearly one fourth (26.96%) were on NSAIDS. Variceal bleeding (52.94%), followed by peptic ulcer bleed (13.43%) were the most common causes of bleed. In subjects undergoing UGIE for dyspepsia, 37.41% revealed no endoscopic lesion followed by gastro-duodenitis (25.01%). Peptic ulcer was cause of dyspepsia in 18.05% and was significantly more in those with alarm symptoms (<0.001). Alarm symptoms in dyspepsia has a significant high likelihood of finding a malignant lesion on endoscopic evaluation (p 0.013).Conclusions: Variceal bleed is the most common cause of UGIB in the adult Indian population. In patients with dyspepsia, presence of alarm symptoms is significantly associated with organic lesion on endoscopy. Although the incidence of malignancy is low, endoscopy in more than 50years subjects presenting with dyspepsia may help in early diagnosis and reducing morbidity.

ABSTRACT

Kawasaki disease (KD) is a systemic vasculitis associated with various clinical manifestations and complications, such as gastrointestinal abnormalities. We report a 3-year-old boy who presented with hematemesis and diffuse gastroduodenal ulcerations complicating KD. He received standard medical therapy for the disease and gastric ulcer, which showed effect after a few days. Although rare, peptic ulcers should be considered a complication of KD to ensure early diagnosis and treatment as it may cause severe morbidity.

ABSTRACT

El sangrado digestivo alto (SDA) se define como la presencia de sangre en el tracto digestivo alto, proveniente de la mucosa o vasos sanguíneos que se localizan entre el esófago y el ángulo de Treitz. Las principales manifestaciones clínicas asociadas al SDA están dadas por la presencia de hematemesis, melena y en algunos casos hematoquezia, esta última relacionada con sangrados que cursan con aceleración del tránsito intestinal. En el Hospital de Especialidades Carlos Andrade Marín el SDA es un motivo frecuente de consulta en urgencias, que implica además un importante número de ingresos tanto al área crítica de Urgencias, Unidad de Cuidados Intensivos y hospitalización. La mortalidad por esta causa es variable, se estima en 3,5% al 10,0% y esta depende sobre todo de la edad del paciente y sus posibles enfermedades asociadas, la cuantía del sangrado y los hallazgos que se encuentren durante la realización del procedimiento endoscópico. El sangrado digestivo alto es autolimitado o cesa espontáneamente en el 80,0% de los casos. La causa más frecuente es la hemorragia por úlcera péptica (25,0% ulcera duodenal­23,0% ulcera gástrica). Con este protocolo clínico buscamos normar el manejo hospitalario de los pacientes que ingresan al área de Urgencias con el fin de unificar criterios que conlleven al manejo adecuado de los pacientes con sangrado digestivo alto.


Upper digestive tract bleeding (SDA) is defined as the presence of blood in the upper digestive tract, coming from the mucosa or blood vessels that are located between the esophagus and the Treitz angle. The main clinical manifestations associated with SDA are given by the presence of hematemesis, melena and in some cases hematochezia, the latter related to bleeding that occurs with the acceleration of intestinal transit. In the Hospital of Specialties Carlos Andrade Marín, the SDA is a frequent reason for consultation in the emergency room, which also implies an important number of admissions to the critical area of Emergency, Intensive Care Unit and hospitalization. Mortality from this cause is variable, it is estimated at 3,5% to 10,0% and this depends above all on the age of the patient and their possible associated diseases, the amount of bleeding and the findings that are found during the performance of the endoscopic procedure. High digestive bleeding is self-limiting or ceases spontaneously in 80,0% of cases. The most frequent cause is peptic ulcer hemorrhage (25,0% duodenal ulcer -23,0% gastric ulcer). With this clinical protocol, we seek to regulate the hospital management of patients who enter the Emergency Department to unify criteria that lead to the proper management of patients with high digestive bleeding.

El dolor torácico es un motivo frecuente de consulta en los servicios de urgencias, sin embargo, el hematoma esofágico es una causa muy poco frecuente. Se manifiesta más frecuentemente en mujeres de edad media y avanzada, con una triada de dolor torácico súbito, disfagia u odinofagia y hematemesis. Presentamos un caso de una mujer de 89 años con antecedentes de hipertensión arterial en tratamiento, usuaria de antihipertensivos y aspirina, con una historia de dos días de evolución de dolor retroesternal, sensación febril, disnea y tos productiva secundario a un hematoma esofágico.


Thoracic pain is a frequent reason for consultation in the emergency department, however, esophageal hematoma is a very rare cause. It manifests more commonly in women of middle and advanced age, with a triad of sudden chest pain, dysphagia or odynophagia and hematemesis. We present a case of a 89 years old female patient, with a history of arterial hypertension in treatment, user of antihypertensive drugs and aspirin with a two day history of retrosternal pain, febrile sensation, dyspnea and productive cough due to a esophageal hematoma.

Las lesiones de Dieulafoy se definen como una anomalía vascular que producen un sangrado en la mucosa proveniente de una arteria submucosa anormalmente larga y tortuosa que se ubican principalmente en estómago (80%), con poca frecuencia a nivel duodenal y constituyen el 6% de los sangrados gastrointestinales no asociados a varices y del 1% al 2% de todos los sangrados gastrointestinales. Reportamos el caso de un paciente masculino de 60 años que ingresó por hematemesis, melena y signos de hipovolemia con hemoglobina de 9 g/dl, que luego de realizar su compensación hemodinámica se le realizó una endoscopía alta que evidenció una lesión de Dieulafoy con sangrado activo severo en la segunda porción duodenal realizándosele inyección con adrenalina al 1:10 000 sobre los bordes de la lesión, pero al no lograrse una hemostasia adecuada se le tuvo que realizar la colocación de un clip ovesco con hemostasia efectiva que se corroboró 24 horas después, a través de una segunda endoscopía para poder egresar al paciente sin complicaciones, ni resangrado.


Dieulafoy's lesions are vascular anomalies that produce gastrointestinal bleeding of the mucosa from an abnormally long and tortuous submucosal artery. It is found predominately in the stomach (80%) and less frequently in the duodenum. They constitute the 6% of all non-variceal bleeding and the 1 to 2% of all gastrointestinal bleeding source. We report a case of a 60-year-old man with hematemesis, melena and signs of hypovolemic shock with a hemoglobin level of 9 g/dL. After adequate fluid resuscitation, an upper endoscopy showed an actively bleeding Dieulafoy lesion in the second portion of the duodenum. After epinephrine injection over lesion borders, an adequate hemostasis was not achieved. An over-the-scope clip was placed. Follow-up endoscopy 24 hours later showed an effective hemostasis and the patient was discharged without complications or re-bleeding.

ABSTRACT

El coriocarcinoma gástrico primario (CGP) es un tumor extremadamente raro, altamente invasivo y de rápida diseminación hematógena. Presentamos el caso de una paciente de 57 años que inicia con cuadro de hematemesis y, progresivamente, se le suman episodios de melena, baja de peso y epigastralgia. Es derivada al Instituto Nacional de Enfermedades Neoplásicas en donde se le realizan gastroscopía y biopsia. Así, el análisis histológico reportó patrón sugestivo para CGP; el cual se confirmó al realizarle a la paciente los estudios por imágenes necesarios y llevar a cabo el análisis inmunohistoquímico para gonadotrofina coriónica humana y alfa feto proteína. Posteriormente, a la paciente se le realiza una gastrectomía radical D2 con preservación esplénica y de cola de páncreas. Lamentablemente, su evolución no fue favorable y fallece por la progresión de la enfermedad.


Primary gastric choriocarcinoma (PGC) is an extremely rare and highly invasive tumor with rapid hematogenous spread. We present the case of a 57-year-old female patient who started with hematemesis and progressive episodes of melena, weight loss and epigastralgia. It is derived from the National Institute of Neoplastic Diseases where gastroscopy and biopsy are performed. Histological analysis reported pattern suggestive of PGC; that was confirmed by immunohistochemical analysis for human chorionic gonadotrophin and fetal alpha protein. Subsequently, the patient underwent a radical D2 gastrectomy with splenic preservation and tail of the pancreas preservation. Unfortunately, her evolution was not favorable and died due to the progression of the disease.

Introducción: el sangrado digestivo alto constituye uno de los síndromes más frecuentes en servicios de urgencias. La endoscopia oral y los medicamentos antiácidos y protectores de la mucosa gástrica, hacen menos frecuente el empleo de intervenciones quirúrgicas en el manejo del paciente con sangrado digestivo alto. La necesidad del tratamiento quirúrgico no siempre puede obviarse. Objetivo: describir las características clínicas de los pacientes con sangrado digestivo alto atendidos en el Hospital Militar Dr. Joaquín Castillo Duany. Métodos: estudio descriptivo transversal. Universo constituido por 92 pacientes con diagnóstico de sangrado digestivo alto. Variables caracterizadas: edad, sexo, tratamiento médico empleado, técnica quirúrgica utilizada, complicaciones posquirúrgicas y estadía hospitalaria. Resultados: el sangrado digestivo alto fue más frecuente en hombres, con más de 40 años de edad, se les aplicó tratamiento médico, la modalidad más frecuente fue la combinación de antiácidos. Se realizó tratamiento endoscópico al 3,2 por ciento de los pacientes, fueron intervenidos quirúrgicamente el 5,4 por ciento. La mitad de los operados padecía úlcera péptica gástrica. La técnica quirúrgica más empleada fue la gastrostomía y gastrorrafia. La estadía hospitalaria fue menor de 5 días. Conclusiones: se evidenció predominio del sangrado digestivo alto en el sexo masculino y en mayores de 40 años. El tratamiento endoscópico y las intervenciones quirúrgicas representaron un bajo porcentaje. La estadía hospitalaria fue más prolongada en pacientes con complicaciones. Los hallazgos en los que más difieren otras investigaciones radican en el incremento del empleo de la endoscopia oral, en combinación con el uso de antisecretores y en las variantes de técnicas quirúrgicas empleadas(AU)


Introduction: upper Digestive Bleeding is one of the most frequent syndromes in emergency services. Oral endoscopy, antacid and protective gastric mucosal medications make the use of surgical interventions less common in the management of upper digestive bleeding patients. However, the need for surgical treatment cannot always be overlooked. Objective: to describe the clinical characteristics of patients with upper digestive bleeding treated at the Military Hospital Dr. Joaquín Castillo Duany. Methods: cross-sectional descriptive study. A universe of 92 patients with a diagnosis of upper digestive bleeding. Variables characterized: Age, sex, medical treatment, surgical technique used, postoperative complications, and hospital stay. Results: upper digestive bleeding was more frequent in men and in patients over 40 years old. In patients in whom medical treatment was applied, the most frequent modality was the combination of anti-H2 antihistamines and proton pump inhibitors. Endoscopic treatment was performed in 3.2 percent of patients, and 5.4 pèrcent was operated on. Half of the patients underwent gastric peptic ulcer, and the most commonly used surgical technique was gastrostomy. The predominant hospital stay was less than 5 days(AU)

ABSTRACT

Hemorragia digestiva alta (HDA) é uma emergência clínica exteriorizada através de melena, hematêmese, além de alterações hemodinâmicas decorrentes de perda volêmica. Existem vários diagnósticos etiológicos causadores de hemorragias digestivas altas; dentre eles, uma causa rara é lesão de Dieulafoy, sendo relacionada acerca de 2% dos casos de HDA. Neste relato, descreveremos um caso de lesão de Dieulafoy, evidenciando a relevância desta lesão como um importante diagnóstico diferencial nas causas de hemorragia digestiva alta. O procedimento diagnóstico e o manejo atual são fundamentados pela endoscopia digestiva alta. A hemostasia endoscópica é realizada, utilizando técnicas como eletrocoagulação, injeções com adrenalina e hemoclips.


Upper gastrointestinal bleeding (UGBI) is a clinical emergency externalized through melena, hematemesis and hemodynamic changes due to volume loss. There are a lot of etiological diagnosis of gastroduodenal hemorrhages, including Dieulafoy which is a rare cause related about 2% of the UGBI cases. In this report we describe a case of Dieulafoy lesion highlighting the importance of this lesion as a rare and important differential diagnosis in cases of upper gastrointestinal bleeding. The current diagnostic and management is justified by endoscopy. Endoscopic hemostasis is performed using techniques such as electrocoagulation, injections of adrenaline and hemo-clips.

Introducción: la hemorragia digestiva alta constituye un problema frecuente de salud en el mundo; así se comporta en Cuba, en la provincia Matanzas y en el hospital de estudio. Actualmente es considerada como causa mayor de morbimortalidad. Objetivo: determinar el comportamiento de la hemorragia digestiva alta en el Hospital Militar de Matanzas. Materiales y Métodos: se realizó una investigación descriptiva, en un periodo de cinco años que incluyó a todos los pacientes ingresados con ese diagnóstico, y los que durante su ingreso por otra causa presentaron episodios de hemorragia. El índice de Rockall permitió evaluar la necesidad de cirugía, recidiva y mortalidad. Resultados: la mayoría de los pacientes pertenecieron al grupo de riesgo intermedio. Los hombres fueron los más afectados, el grupo de tercera edad presentó mayor incidencia. La gastritis hemorrágica fue la primera causa de sangrado. La hipertensión arterial resultó ser el factor de riesgo relevante y el tabaquismo el hábito tóxico más frecuente. Conclusiones: como beneficio relevante se demostró la importancia de la endoscopia precoz para el diagnóstico de la causa, tratamiento y pronóstico de la hemorragia. Se observó una buena correlación entre el pronóstico del paciente al ingreso y su estado al final del tratamiento (AU).


Introduction: the high digestive hemorrhage is a frequent health problem in the world; it behaves the same in Cuba, in the province of Matanzas and in the hospital where the study was carried out. Currently it is considered a major cause of morbi-mortality. Objective: to determine the behavior of the high digestive bleeding in the Military Hospital of Matanzas. Materials and Methods: a descriptive research was carried out in a five-year period. It included all the patients admitted with that diagnosis and those who presented episodes of hemorrhage even if they were admitted by any other cause. The Rockall index allowed to assess the necessity of surgery, the recidivism and mortality. Results: most of the patients belonged to the intermediate risk group. Male patients were the most affected one; the group of elder people showed higher incidence. Hemorrhagic gastritis was the first cause of bleeding. Arterial hypertension was the relevant risk factor and smoking the most frequent toxic habit. Conclusions: as a relevant benefit it was showed the importance of precocious endoscopy for the diagnosis of the hemorrhage cause, treatment and prognosis. It was observed a good correlation between the patient´s prognosis at the admission and his status at the end of the treatment (AU).

ABSTRACT CONTEXT: Boerhaave syndrome consists of spontaneous longitudinal transmural rupture of the esophagus, usually in its distal part. It generally develops during or after persistent vomiting as a consequence of a sudden increase in intraluminal pressure in the esophagus. It is extremely rare in clinical practice. In 50% of the cases, it is manifested by Mackler's triad: vomiting, lower thoracic pain and subcutaneous emphysema. Hematemesis is an uncommon yet challenging presentation of Boerhaave's syndrome. Compared with ruptures of other parts of the digestive tract, spontaneous rupture is characterized by a higher mortality rate. CASE REPORT: This paper presents a 64-year-old female patient whose vomit was black four days before examination and became bloody on the day of the examination. Her symptoms included epigastric pain and suffocation. Physical examination showed hypotension, tachycardia, dyspnea and a swollen and painful abdomen. Auscultation showed lateral crackling sounds on inspiration. Ultrasound examination showed a distended stomach filled with fluid. Over 1000 ml of fresh blood was extracted by means of nasogastric suction. Esophagogastroduodenoscopy was discontinued immediately upon entering the proximal esophagus, where a large amount of fresh blood was observed. The patient was sent for emergency abdominal surgery, during which she died. An autopsy established a diagnosis of Boerhaave syndrome and ulceration in the duodenal bulb. CONCLUSION: Boerhaave syndrome should be considered in all cases with a combination of gastrointestinal symptoms (especially epigastric pain and vomiting) and pulmonary signs and symptoms (especially suffocation).


RESUMO CONTEXTO: A síndrome de Boerhaave é uma ruptura longitudinal transmural espontânea do esôfago, normalmente da parte distal. Ela geralmente se desenvolve durante ou após vômitos persistentes como consequência do aumento repentino da pressão intraluminal no esôfago. É extremamente rara na prática clínica. Em 50% dos casos, manifesta-se pela tríade de Mackler: vômitos, dor torácica inferior, enfisema subcutâneo. Hematêmese é uma apresentação incomum porém desafiadora da síndrome de Boerhaave. Em comparação com rupturas de outras partes do tubo digestivo, a ruptura espontânea é caracterizada pela taxa de mortalidade mais elevada. RELATO DO CASO: O artigo apresenta uma paciente do sexo feminino de 64 anos de idade, cujo vômito era preto, quatro dias antes do exame, e continha sangue no dia do exame. Os sintomas incluíam dor epigástrica e sufocação. No exame físico, foi verificada hipotensão, taquicardia, dispneia e abdômen inchado e doloroso. Ausculta revelou estertores laterais na inspiração. A ultrassonografia mostrou estômago dilatado, preenchido com conteúdo líquido. Sucção nasogástrica evacuou mais de 1.000 ml de sangue fresco. Esofagogastroduodenoscopia foi abortada imediatamente ao se entrar no esôfago proximal, onde foi observada grande quantidade de sangue fresco. A paciente foi encaminhada com urgência para cirurgia abdominal, durante a qual faleceu. Autópsia estabeleceu diagnóstico de síndrome de Boerhaave e úlcera no bulbo-duodenal. CONCLUSÃO: A síndrome Boerhaave deve ser considerada em todos os casos com uma combinação de sintomas gastrointestinais (especialmente dor epigástrica e vómitos) e sintomas e sinais pulmonares (especialmente sufocação).

Back ground: It is highly unfortunate that many patients are admitted daily with hematemesis and or malena due to the adverse effects of drugs either prescribed or self-medicated. Incidence of such cases can be greatly reduced if medical practitioners are not only aware of the adverse effects of drugs on gastrointestinal tract but also assess the patients for the risk factors of drug-induced UGI bleeding before prescribing these drugs and also by properly educating the patients. Aim: To study the clinical profile and risk factors in fifty cases of drug induced UGI bleeding. Materials and methods: Fifty patients (32 males and 18 females) admitted with drug-induced hematemesis and or malena were studied with respect to age group, number of bouts of hematemesis, approximate quantity of total blood loss, causative drug responsible for UGI bleeding, risk factors of GI bleeding, number of individual risk factors in each patients. The prevalence of individual risk factors in those fifty patients and the prevalence of number of risk factors in those fifty patients were studied. Thorough clinical and laboratory investigations were done. Results: Hematemesis was the common symptom. Ibuprofen was responsible for the majority of cases (38%) followed by diclofenac (22%). Prevalence of risk factors among the patients are as follows: Age > 50 years of age - 66%, ‘O’ Blood group – 50%, Alcoholism – 42%, Not using Gastro protective agents – 40%, Self-medication / OTC drugs – 36%, Smoking – 30%, Stress and Serious systemic illnesses – 12%, Helicobacter pylori – 12%, Known Peptic ulcer disease – 10% , High doses/ Chronic drug intake – 10%, Concomitant use of Steroids – 8% and Concomitant use of anticoagulants – 4%. Conclusion: NSAIDs were the commonest cause for UGI bleeding. Age > 50 years (66%) was the commonest risk factor for UGI bleeding. All those fifty cases had at least one known risk factor and majority (80%) had more than one risk factors of drug-induced UGI bleeding

Background: Non-steroidal anti-inflammatory drugs (NSAIDs) including aspirin are among the most frequently prescribed drugs worldwide and are available ‘Over-The-Counter’ (OTC) also. Though reasonably safe in most cases in prescribed dosages and for short durations, these drugs cause serious gastrointestinal toxicity in a large number of cases. They can affect all segments of the gastrointestinal tract causing ulcers, severe bleeding, perforation, and obstruction. Aim: To study the endoscopic abnormalities in fifty cases of drug induced UGI bleeding. Materials and methods: Fifty patients (32 males and 18 females) admitted with drug-induced hematemesis and or malena were studied with respect to age group, number of bouts of hematemesis, approximate quantity of total blood loss, causative drug responsible for UGI bleeding, risk factors of GI bleeding, number of individual risk factors in each patients. Thorough clinical and laboratory investigations were done. UGI scopy was done for all patients. Results: NSAIDs were the commonest cause for UGI bleeding. Age > 50 years (66%) was the commonest risk factor for UGI bleeding. It was observed that ten patients were having normal endoscopic study, twenty five patients were having lesions in stomach only, ten patients were having lesions in duodenum only and five were having lesions in both stomach and duodenum. None of the patients studied, had lesions in the esophagus. It was found that ulcers were more common than erosions. Conclusion: Stomach is the commonest site affected (50%) and ulcers (64%) were common than erosion.

ABSTRACT

Dieulafoy lesions, vascular anomalies typically found along the gastrointestinal tract, have been viewed as rare and obscure causes of sudden intestinal bleeding, especially in pediatric patients. Since their discovery in the late 19th century, the reported incidence has increased. This is due to an increased awareness of, and knowledge about, their presentation and to advanced endoscopic diagnosis and therapy. Our patient was a three-year-old male, without a complex medical history. He presented to the emergency department with acute hematemesis with blood clots and acute anemia requiring blood transfusion. Endoscopy revealed four isolated Dieulafoy lesions along the lesser curvature of the stomach, which were treated with an epinephrine injection. The Dieulafoy lesion, although thought to be rare, should be considered when investigating an acute gastrointestinal bleed. These lesions have been successfully treated endoscopically. Appropriate anticipation and preparation for diagnosis and therapy can lead to optimal outcomes for the pediatric patient.
